Keystone Wellness Sliding Fee Scale
At Keystone Wellness we believe that everyone should be able to afford the care they need. In order to achieve this we ask that all of our clients provide financial information within the enrollment process so that we can adjust fees to meet financial need. Our sliding fee scale is built on the Federal Poverty Guidelines and adjusts fees based on a combination of client household size and income data. Medicaid for example uses these guidelines for their eligibility criteria, and in 2025 generally adults under 65 must be at or below the 138% level on this chart to qualify for medicaid.

The Keystone Wellness Sliding Fee Scale works as follows. Our office staff uses the information provided by the client to determine what percentile the client matches on the federal guidelines. This is then rounded down to the nearest entry on the table below, and all fees billed to the client (including co-pays) are adjusted as indicated.
