Daily Activity Log Instructions

  1. Open the survey via either the QR code or link at the right.

  2. Once there you will enter your name, email, and the date of the activity. You may find that you want to save the link to this form and have your phone auto-populate your name and email. The form is set up to automatically populate today's date in the date field, so if you are logging your activity on the same day you did it you won't need to do anything in this field.

  3. Upload an image of your steps/activity for the day. We want you to engage in the activity that works best for you and to use the tracking method that works best for you. If you use a pedometer you can take a picture of that. If you use wearable technology you can upload a screenshot of your app screen. If you walk keep walking. If you swim keep swimming. If you are using PokemonGO to be active you can take a screenshot of the route you walked. You use the method that works for you and upload a picture that shows us what you are doing. If you have questions reach out to activities@keystonewellnessmt.org.

  4. In the activity comment section please tell us what brought you joy during your daily activity today. Whether you were walking, jogging, hiking or doing a PokemonGo route we want to know how the time you spent engaged in this activity sparked joy for you.

To log your daily activity, you will go take the following steps:

Weekly Virtual Event Instructions

General Weekly Activity Log Instructions

When logging weekly activities you will use the same link as you use for your daily activity log, however, you will use the photo and comment instructions below, and you are only able to post one time per week in each weekly category.

Specific Weekly Activity Log Instructions

Find The Keystone QR Code

Each week the Keystone Staff will hide a Keystone Logo around town with a QR Code on it. At the beginning of the week we will post a clue giving Walk-a-thon participants a hint about where the Keystone QR code is hidden this week. When you find it, scan it on the Activity Log and for the picture post a selfie of yourself with the Keystone QR code. For the comment please tell us how you used the clue to find the code.

Elevation Challenge

Each week you can earn one point by submitting the activity log with a screenshot from your smart device showing your highest elevation reached by FOOT or BIKE. For your comment on this log you should tell which trail you hiked or rode to get to this elevation and what the view was like when you you were there.

Water Based Challenge

Each week you can earn one point by submitting the activity log with a selfie of showing your water based activity on a kayak, canoe, paddleboard, or other watercraft that you rowed yourself. For your comment on this log you should tell us where you went and what your favorite part of your water activity was.

Bike Based Challenge

Each week you can earn one point by submitting the activity log with a screenshot or selfie representing your bike ride. For your comment on this log you should tell us where you went and what your favorite part of your bike activity was.

Note - you may not use the same activity for the bike based challenge and the elevation challenge. If you use a bike ride for the elevation challenge, your bike based challenge log must come from a different bike ride that week.
